Whole Food Plant Based Diet for Diabetes (DrRic Question from Ben)

A whole-foods, plant-based eating pattern generally includes legumes, whole grains, fruits, vegetables, and nuts, and is high in fiber. All of these elements have been found to be protective against diabetes. Whole grains, including whole-grain bread, whole-grain cereals, and brown rice, have been associated with reduced risk of developing diabetes , a recent systematic review and meta-analysis of 16 cohort studies found a summary relative risk of 0.68 for three daily servings of whole grains. Specific fruits and vegetables, including root vegetables, green leafy vegetables, blueberries, grapes, and apples, have been linked to lower diabetes rates., Legumes have also been shown to ameliorate insulin resistance and protect against metabolic syndrome, and greater nut consumption has been associated with lower diabetes risk. Cereal fiber appears to be especially protective against type 2 diabetes.

Diabetes Risk Factors And Causes

There can be several causes of diabetes, especially type II diabetes. Usually, its a combination of genes and lifestyle choices. Eating bad foods and eating a lot of them is usually the culprit.

A 2014 study about obesity and diabetes found a direct link between the two. It noted that an increase in body mass index could lead to insulin resistance. Obesity in young people can also lead to type I diabetes.

How Is Diabetes Diagnosed

There are two tests that help diagnose diabetes: fasting blood sugar tests and glycated hemoglobin tests . The latter does not require fasting and is performed in a lab. Its also more conclusive than a simple fasting blood sugar test.

If you have a blood glucose level of 126 ml/dl or over after fasting, then youre likely diabetic. Similarly, if your random blood sugar levels are 200 or higher, youre diabetic.

For the HbA1c test, a result of 6.5% or higher indicates diabetes. This test is an indicator of your blood sugar levels on average for the past two or three months.

According to the Mayo Clinic, some of the symptoms of type II diabetes are fatigue, frequent urination, blurred vision, frequent infections, and increased thirst.

Reduces Blood Sugar Level

As a diabetic person, you have to manage your blood glucose levels at all times. This means that such people do not enjoy the liberty of eating all kinds of foods. Vegetables, fruits, nuts, and seeds mostly have a low glycemic index and good for those with diabetes.

A study conducted by the Physicians Committee for Responsible Medicine revealed that a plant-based diet is three times better at controlling blood sugar than a regular diabetes diet. The participants showed improvement in their insulin sensitivity.

The Cons Of A Vegan Diet For Type 2 Diabetes

Plant

Experts agree that, for a vegan diet to be effective, it must be carefully thought out. French fries could be vegan, or we can eat a vegan cupcake but both still have sugars and a high number of carbs, Chaparro says. The key words well-planned, well-balanced, and a nutritious vegan diet.

Focusing on foods like green leafy vegetables, onions, mushrooms, beans, berries, and nuts, and seeds which are rich in phytochemicals and antioxidants is ideal, Fuhrman says.

Because you wont be eating meat or dairy, its important to consume enough protein and some healthy fats, to promote a healthy weight and overall health. You cant load your diet just full of starches or carbohydrates, because theyll impact your blood sugar the most, Chaparro says.

Some people, such as those older than age 85 and those who are athletic, might need more protein. Because meat is the richest sources of the easy-to-absorb heme form of iron, people who follow a vegan diet will need to be more mindful of getting enough of this important nutrient from their food, as will those who have a genetic or inherent digestive problem that prevents them from absorbing iron. This is true regardless of whether the person has type 2 diabetes, Fuhrman says.

People who follow a vegan diet are also at risk for nutritional deficiencies, such as calcium, zinc, vitamins B12, riboflavin, and D, and may need to focus on certain foods, or take supplements.

Before starting a WFPB diet, consult your HCP in order to discuss any necessary medication changes due to decreased insulin resistance and weight loss.

Also, make sure you know how to treat a low blood sugar. An abrupt change to eating a WFPB diet may result in a great decrease in insulin resistance, which means that those with diabetes who use oral hypoglycemic agents or insulin may encounter low blood sugar .

Choosing The Best Plant Foods

PREVENT, REVERSE & CURE TYPE 2 DIABETES WITH A WHOLE FOOD PLANT BASED DIET

Plant-based foods include kale chips and sugar snap peas, but also potato chips and sugary fruit drinks. The benefits of plant-based eating come from minimally processed plant foods. The best options include whole grains, nuts, seeds, legumes , fruits, and vegetables.

Nonbeneficial plant-based foods include fruit juices, sweetened beverages, and refined grains. Its wise to limit these foods, whether you have diabetes or not.

How Does A Plant Based Diet Help Type 2 Diabetes

A plant based diet has been shown to not only prevent, but also reverse, type 2 diabetes. A 74 week study showed “a plant-based diet controlled blood sugar three times more effectively than a traditional diabetes diet that limited calories and carbohydrates .”

Participants in this study also saw other health benefits, including weight loss, improved insulin sensitivity, lower HbA1c levels.

Implementation In Clinical Practice

Several reviews have presented practical strategies for using plant-based diets in clinical scenarios, including type 2 diabetes management. The key elements of the eating pattern include avoidance of animal products, highly refined grains, added sugars, and oils, and consumption of an abundance of legumes, leafy greens, cruciferous vegetables, starchy vegetables, whole grains, and fruits. In key randomized clinical trials of plant-based diets, low-glycemic index foods have been encouraged.,

Some clinicians may assume that their patients will not be open to adopting a plant-based diet. However, a plant-based diet has been shown to be similarly acceptable to an ADA diet among people with diabetes plant-based diets have also been found to be highly acceptable in other medical contexts. Common questions about specific macro- and micronutrients in plant-based diets have been addressed elsewhere.,,, Patients who adopt a plant-based diet can experience decreases in blood sugar and blood pressure relatively quickly after changing their diet, especially if they are taking medications for these conditions. Close monitoring and anticipation of hypoglycemia is critical medications may require adjustment. Ongoing support, education, and follow-up can help patients achieve and maintain dietary changes.

Improving Blood Sugar Control To Promote A Healthy Weight

Plant Based Diet for Diabetes: How to use plant based diet ...

In the study, originally published in February 2018 in Nutrients, Dr. Kahleova and her colleagues followed 75 overweight and obese adults without diabetes whom they had randomly assigned to a vegan diet or a control diet for a 16-week period.

The vegan group ate meals composed of vegetables, grains, legumes, and fruits, and they limited their daily fat intake to about 1 ounce or less. Meanwhile, the control group maintained their current diets, which included meat and dairy. Neither group restricted their calories.

Subjects had to complete a three-day dietary record at the start and finish of the study to demonstrate that they were sticking to their assigned eating plan.

Although participants were diabetes free, they all had high body mass indexes of 28 to 40, which put them at high risk for developing type 2 diabetes. At the beginning of the study, about one-third had prediabetes, meaning their blood sugar levels were higher than normal but not high enough to signify type 2 diabetes.

Insulin resistance is high in overweight people without diabetes, exerting high demands on beta cells to produce insulin, says Kahleova. By the time type 2 diabetes is diagnosed, about 50 percent of the beta cells are already dead. Therefore, the greatest opportunity to preserve beta-cell function is before diabetes onset.

Researchers speculate that a diet high in plants and low in animal products may produce these positive results by reducing oxidative stress and inflammation.

Unveiling The Power Of A Plant

This study is great because it shows that nutritionally you may be able to prevent type 2 diabetes, says Audrey Koltun, RD, a certified diabetes educator with Cohen Childrens Medical Center in New Hyde Park, New York.

Kristin Kirkpatrick, RD, the manager of wellness nutritional services at the Cleveland Clinic, says this investigation supports previous research showing an association between plant-based eating plans and a reduction in diabetes risk. For example, a review of three studies published in June 2016 in PLoS Medicine suggests that cutting back on meat and dairy, and increasing fruit, veggie, nut, and seed intake, is linked to a substantially lower risk for type 2 diabetes.

Even starting small can help, says Kirkpatrick, who was not involved in the research. Choose one or two days a week that will just be plant-based. Make more effort to get natural color in your diet, and try to omit all processed red meat, such as bacon, sausage, deli meat, and hot dogs, per the World Health Organization, which notes that these foods are also associated with a higher risk for cancer.

Yet Koltun, who wasnt involved in the study, warns that not all vegan foods are good for you. French fries, chips, and white bread are all vegan foods, she says. You have to follow a healthy vegan diet with real fiber from whole grains, nuts, and legumes. These foods help slow down how fast a meal is digested, therefore requiring less abundance of insulin to be used.

Adjusting Medications In Response To A Plant

It is essential to help patients anticipate blood glucose changes in response to the new eating pattern. Hypoglycemia is common among individuals treated with insulin or sulfonylureas as they improve their diets, often necessitating medication reduction or discontinuation. There is a need for the development of evidence-based deprescribing guidelines for patients with type 2 diabetes who need to reduce medication in response to episodes of hypoglycemia in the context of a plant-based diet.

How Can I Lose Weight If Im A Vegetarian

Following a vegetarian diet can be healthy, but if you simply replace meat and fish with processed foods, which can be high in calories and low in vitamins and minerals, eg ready meals, pastries, pies, takeaways, cheese, creamy sauces and dips, your diet may be unbalanced and youre likely to put on weight. That is why it is important to replace such high-fat foods like these with cottage cheese, eggs, dishes based around beans, tomato-based sauces, and nuts and seeds in moderation, and fill up on vegetables served with wholegrain rice, pasta or bread. It is extremely important to watch your overall portions. Keeping active is also important for weight management.

Limit Highly Processed Snacks

Don’t be fooled: vegan highly processed snacks are still highly processed snacks. Try to stick to whole foods whenever you can, like celery or fruit with nut butter, pita with hummus or roasted chickpeas, which are all great for curbing hunger and keeping you feeling energized until your next meal. Nuts and seeds are a great grab-and-go snack in a pinch, and they are shelf stable so storing them is a breeze. Try to pair a complex carb with a source of protein and fiber for a well rounded snack.

If you want to prep ahead, we have plenty of plant-based snack recipes like our Savory Date & Pistachio Bites, Garlic Hummus, Everything Bagel Microwave Popcorn and Strawberry-Pineapple Smoothie. Make a batch in advance to get ready for the week ahead.

That being said, there are plenty of healthy packaged snack options out there, and the convenience just can’t be beat when you’re in a pinch. When shopping, be sure to look at nutrition labels and go for snacks that have the least amount of added sugar, sodium and saturated fat per serving.

Benefits For Type 1 Diabetes

Plant

OK, hold on here perhaps this works for prediabetes or even type 2, but what about type 1? You cant just eliminate the need for insulin. So is there really any true benefit for those with T1D?

Trapp says yes.

As a leading U.S. expert on this topic, shes not aware of any specific studies focusing on plant-based eating and T1D effects. But she says shes heard a lot of anecdotal evidence through the years.

Ive learned from my patients that a plant-based diet is doable and powerful. It can help to improve insulin sensitivity. In fact, Ive had patients get angry that no one told them about this earlier. So now I offer it as an option to all.

Most importantly, Trapp says diabetes complications are a key in adopting this type of diet plan for PWDs.

Consider that complications of type 1 are largely vascular, so the same artery-protecting plant-based foods would be beneficial, she points out. In my own experience, insulin needs can be reduced in T1. Absolutely worth a try.

Trapp is also quick to note that insulin-to-carb ratios vary and can change, so a healthcare teams support is crucial as one navigates this type of eating. She points to the California-based consultancy Mastering Diabetes, that has embraced this lifestyle, as well as a resource from the Physicians Committee that includes a four-page handout on plant-based diets for those with T1D.
