Its Fair That Pharma Should Be Able To Cover Their Production Costs
Through their lawsuits, the Type 1 Diabetes Defense Foundation reversed engineered the production costs of analog insulins. The costs would be just as covered if they were priced similarly to the human insulin available at Walmart.
The production cost of biosynthesized insulins should logically have decreased as manufacturers simplified and optimized production processes since the 1970s. The current production cost for insulin crystals has been reported as low as $42.20 per gram. According to this figure, the estimated production cost for the 35 mg of insulin crystals contained in a standard 10 ml vial at a concentration of 100 IU would not exceed $1.47. Such a production cost would be consistent with 1996 list prices for synthetic analog insulins, $24 per 10 ml vial. It does not explain why list prices per 10 ml vial of the same synthetic analog insulins would increase to current levels .
Additionally, a study published in BMJ Global Health estimated the cost of to produce a vial of human insulin is between $2.28 and $3.42, and analog insulin between $3.69 and $6.16.
Surviving And Thriving On R And Nph From Walmart

How Many Vials Of Insulin Do People With Diabetes Need
Most types of insulin come in 10 milliliter vials and contain 1,000 units of insulin. While dosage varies from person to person, a vial of this size will typically facilitate 20 to 30 injections. Insulin pens usually contain 300 units of insulin and facilitate about 6 to 10 injections.
People with Type 1 diabetes usually start with two injections of two different types of insulin per day and generally progress to 3-4 injections of different types per day. Most people with Type 2 diabetes may need one injection per day without any diabetes pills. Some may need a single injection of insulin in the evening along with diabetes pills. Sometimes diabetes pills stop working, and people with Type 2 diabetes may progress from single to 3-4 injections of insulin per day.
Walmarts private-brand insulin will cost $72.88 per vial and $85.88 per FlexPen for people without insurance. This may benefit people who have no health insurance or have a high deductible for medications.
While Walmarts short-acting insulin is an affordable alternative, getting on a health insurance plan with a low deductible is the most cost-sustainable option for people who have access to one, Redmond says. Further, patients with diabetes often have to take both short-acting and long-acting insulins, so they will still need to find and pay for the latter.

Generics And Biosimilars Are Driving Down The Overall Price Of Insulins
Since 2019, the overall retail price of insulins has declined by about 5%. Most of this decline can be seen as a result of recent approvals of generics and biosimilars.
In 2019, Eli Lilly released the first generic insulin, insulin lispro, the counterpart to the popular rapid-acting insulin Humalog. Since then, the FDA has approved generic versions of Humalog 75/25 , , , and most recently, Lantus .
Retail prices for generic insulin lispro and insulin aspart are currently about half that of Humalog and Novolog, respectively. The same goes for the generic mixed insulins, insulin lispro 75/25 and insulin aspart 70/30, compared to Humalog 75/25 and Novolog 70/30, respectively.
Generics have been instrumental in bringing down costs for some patients. Instead of paying a retail price of over $700 for a package of 5 Humalog KwikPens, for example, patients can now pay closer to $300 for a package of 5 generic insulin lispro KwikPens. However, weve seen that limited insurance coverage can prevent some patients from accessing these savings.
Like generics, follow-on products have also helped to bring down insulin prices overall. Generics contain the exact replicas of the active ingredients in a brand-name medication. But biologic drugs like insulin are nearly impossible to recreate. So sometimes, manufacturers make close copies of them instead, known as follow-ons or biosimilars. Biosimilars tend to be more expensive than generics but less expensive than brands.
Why People Sometimes Turn To Otc Insulin For Blood Sugar Control
Dr. Goldstein decided to research OTC insulin because she prescribes ReliOn brand insulin as the least expensive option for people without health insurance in her clinical practice. Insulin prices have risen dramatically in recent years, nearly tripling from 2002 to 2013 according to an article published in April 2016 in the Journal of the American Medical Association . In March 2019, Eli Lilly announced it would sell a cheaper generic version of its Humalog 100 insulin for about $137 a vial, effectively cutting the price of the original in half. Other fixes include Cigna and Express Scripts offering a $25 cap on out-of-pocket insulin costs for its consumers in participating plans.
Jennifer Trujillo, PharmD, CDE, associate professor at the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences in Aurora, says she also has patients ask about cheaper options. I think we have to accept the fact that some patients dont have an option but to use the least expensive option available, says Dr. Trujillo. For some patients, this is the only alternative they have.
Besides people without health insurance, Trujillo says that people with high copays or high-deductible plans might be considering OTC insulin. People on Medicare in the donut hole, a coverage gap where patients are often pay more for drugs, might be paying more out-of-pocket for insulin and looking for relief.

Walmart’s New Insulin Still Too Expensive Advocates Say
The retail giant Walmart has launched its first private brand of analog insulin in the United States, called ReliOn , but some advocates worry the lower-cost drug is still too expensive for many.
The products will be available at Walmart pharmacies this week and at Sam’s Club pharmacies across the U.S. by mid-July. Like the brand-name Novolog, they will also be manufactured by Novo Nordisk and will require a prescription.
The new product will cost $72.88 per vial and $85.88 for a package of FlexPens. This represents savings of 58% to 75% off the cash price of branded analog products, or up to $101 per branded vial and $251 per FlexPens package, according to the company. Walmart’s other ReliOn insulins â NPH, Regular, and 70/30 mix â are all human insulin rather than analog insulin,and sell for about $25 a vial. The similar apid-acting lispro analog insulin retails for about $150 a vial.
Analog insulin differs from human insulin the former is typically lab-made and is considered to be more effective and easier to administer than human insulin.
Organizations React Positively Yet Caution More Work Is Needed
“Diabetes often comes with high medical costs, estimated around $9,601 per person per year. We welcome all affordable solutions that make diabetes management more accessible to millions of Americans living with diabetes. We encourage everyone to ask their health care provider questions to better understand what the right and affordable treatment is for their unique medical needs,â Tracey D. Brown, chief executive officer of the American Diabetes Association, said in a statement from Walmart
Type 1 diabetes research and advocacy organization, JDRF, applauded the news, saying, in part, that it âwelcomes Walmart’s announcement regarding their plan to offer lower-cost insulin…For people with type 1 diabetes, insulin is a lifesaving drug that no one should ever have to ration or go without. No one should die because they can’t afford their insulin.â
Still,”while today’s announcement is a step toward making insulin affordable for everyone, more needs to be done,â the organization said âJDRF will continue to urgently drive long-term efforts and push for action from manufacturers, health plans, employers, and the government to remove accessibility barriers.”

Walmart announced Tuesday it will start selling private-label insulin this summer at a much lower price for the lifesaving diabetes drug than branded varieties in vials and pens.
The new brand, called ReliOn NovoLog, includes analog insulin vials for $72.88 and $85.88 for a package of prefilled insulin pens.
Walmart says its lower-priced brand will translate into savings of 58 percent and 75 percent, respectively, compared with the current cash price of branded insulin products when bought without insurance.
The product, which is manufactured by Novo Nordisk, is used to control high blood sugar in adults and children with diabetes. Walmart said it will require a doctors prescription.
It will be available in Walmart pharmacies this week, and in Sams Club pharmacies in mid-July, the company said.
More than two decades ago, Walmart launched a human insulin private label brand called ReliOn that costs about $25 per vial, but thats an older formula that some doctors say isnt as effective as the latest offerings.

If somebody is struggling with being able to afford their insulin, they definitely need to have a conversation with their provider as soon as possible because we would never want somebody to ration or do without their insulin, says Kellie Antinori-Lent, RN, CDE, a diabetes clinical nurse specialist at UPMC Shadyside Hospital in Pittsburgh. and president-elect of the American Association of Diabetes Educators.
Schedule a phone call or an appointment to discuss your concerns. She says drug makers also provide assistance programs for insulin. Not taking your prescribed dose of insulin can make you extremely ill, and people have gone to the hospital and even died, says Antinori-Lent.
Antinori-Lent, who works in a hospital setting rather than an outpatient clinic, says that sometimes patients may take newer insulin in the hospital, but then find out at discharge that they cant afford it and they may need to switch to older insulin. Ideally, these discussions about cost can happen well before discharge so patients get the opportunity to learn how these insulins work and how to use them.
Trujillo says: The most important thing is to be honest and upfront about your situation. I think we have a lot of patients who are rationing their insulin or not taking it as prescribed because of cost issues. One in four people with diabetes report having rationed or not used insulin because of cost, according to an article published in January 2019 in JAMA Internal Medicine.

What Other Products Does Relion Offer And The Prices

The most commonly prescribed form of insulin is insulin analogs, which require a prescription and are currently the subject of a controversy over soaring prices.
In early February 2019, amid continued public outcry over the soaring cost of life-saving pharmaceuticals, readers shared a screen-captured image on social media of a local news station’s reporting on the story of a woman who said she bought inexpensive insulin without a prescription at Walmart:
Readers asked us whether this was true. It is true, although you should note Walmart sells human insulin, an older version of the glucose-moderating hormone, whereas most insulin-dependent diabetics are currently prescribed insulin analogs that have evolved to help prevent dangerous swings in blood-glucose levels.

We confirmed with Walmart that the retail chain does indeed sell human insulin without a prescription . That product is Novo Nordisk-manufactured Novolin, which Walmart has branded as ReliOn and sells for $24.88 per vial. Walmart spokeswoman Marilee McInnis confirmed to us that the company has “maintained, through negotiation, the same retail to patients since 2011.” To say that the insulin is sold “over-the-counter” isn’t quite accurate, however, as customers must ask a pharmacist for it.
