The FreeStyle Libre flash glucose monitoring system is designed to liberate patients from the hassles of glucose monitoring.

How it works.

1.APPLY THE SENSOR

The FreeStyle Libre sensor is applied on to the back of the upper arm with a simple, disposable device called an applicator. When the sensor is applied, a thin, flexible and sterile fibre is inserted just under the skin. It is held in place with a small adhesive pad.

2. SCAN SENSOR

To obtain a glucose reading, simply perform a quick, painless 1-second scan of the reader over the sensor. First, the FreeStyle Libre 2 is an integrated Continuous Glucose Monitor . The FDA classifies an iCGM as a CGM that can be used as part of an integrated system with other compatible medical devices, software, and electronic interfaces. Examples include automated insulin dosing systems, insulin pumps, blood glucose meters or other electronic devices used for diabetes management. Its important to note that Abbott states this system must not be used with an automated insulin dosing system, such as a closed-loop insulin delivery system, and insulin suspend systems. Youre probably asking yourself, What does all of this mean? It means that for now, the FreeStyle Libre 2 can integrate with software and other medical electronic interfaces but cannot control delivery of insulin through an insulin pump.

Second, the FreeStyle Libre 2 has a customizable audio alarm which alerts you within a minute if you are too high or too low without having to scan. This feature brings the FreeStyle Libre 2 on a level playing field with the Dexcom G6. This alarm is optional, giving you the freedom to turn it on or off. The FreeStyle Libre does not have any alarms and requires you to check your reader to see if youre approaching hypoglycemia or hyperglycemia.

Future Closed Loop Technology

Abbott has signed multiple development agreements with partners allowing its Libre monitor to work with future closed loop, or Automated Insulin Delivery systems.

In June 2020, the Food and Drug Administration granted the Libre 2 a special iCGM designation, meaning it has the potential to work with other pieces of diabetes technology, although unlike the similar designation granted to the Dexcom G6, Libre 2 is not supposed to be used with insulin delivery systems.

Most believe this is because the Libre 2 doesnt offer real-time continuous data as the G6 does, but requires follow-up scanning for Highs or Lows to get an actual reading for treatment. Most likely, the new Libre 3 will receive the full iCGM designation soon after its launch in the U.S.

Drug and device maker Abbott has won approval from the U.S. Food and Drug Administration for its next generation FreeStyle Libre 2 iCGM system for adults and children ages 4 and older with diabetes.

While there are other iCGM systems currently on the market, including an older version of the same product made by Abbott, the continuous glucose monitoring sensor in the FreeStyle Libre 2 has the longest wearing time, at 14 days, notes the Abbott Park, Illinoisbased company. The new platform also allows patients to see real-time blood sugar readings, and set optional alarms that will sound in real-time when glucose levels get too low or too high.

The iCGM designation and the addition of alerts are significant improvements for patients with insulin-reated diabetes, says , an associate professor of psychiatry and neurobehavioral sciences at the University of Virginia School of Medicine in Charlottesville, who researches blood glucose and insulin dynamics.

For the most part, though, the new product is an incremental improvement over the previous version of the same sensor, the FreeStyle Libre, Dr. Breton says.

With the FreeStyle Libre 2, patients can scan the sensor with a handheld reader to see their current glucose levels and trends over the previous eight hours. This system also uses Bluetooth technology to alert users when blood sugar gets too high or too low without needing to scan the sensor.

Patients who get readings that dont match their symptoms or expectations may still need to use a finger stick blood test to help inform treatment decisions, according to Abbott.

Newer-generation products that win FDA approval to be marketed this way need to do a lot more than just communicate with a hand held scanner or with devices that can pump insulin into the body, Breton says.

It also ensures minimum levels of accuracy, the capacity to stream data in real-time to other applications, and a regulatory framework of a class 2 medical device with special control that should allow much faster iterations of the product in the future, Breton says. So-called class 2 medical devices dont require the most stringent FDA regulation or approval processes, and can include products like blood pressure cuffs, condoms, pregnancy tests, and power wheelchairs.

How Does An Insulin Pump Work

An insulin pump removes the need for multiple daily injections, which some people find frees them up and makes life more flexible when it comes to eating food and drinking.

It works by attempting to mimic your pancreas by releasing insulin at certain intervals

It delivers insulin in two main ways. Firstly: a basal rate which is a continuous, small trickle of insulin that acts like your âbackground insulinâ such as Levemir or Lantus. This helps keep your blood sugar stable between means and through the night.

Secondly: a bolus rate. This is equivalent to your âfast acting insulinâ such as your Humalog, Novolog etc. This will be a higher dose correlated to what you need to cover the food you are eating.

Basically, an insulin pump allows you to be super accurate with your blood sugars and can give you complete control with limited fluctuations.

However, in order to get to this stage, you need to be patient, willing to practice and learn, and willing to learn to carbohydrate count.

If you can do all those things, then youâll have a much more sense of freedom with diabetes and you wonât be restricted in meal times, what you eat, drinking, and you can even skip meals if you want.

How Can I Get An Insulin Pump

This completely depends on where you live and what your health care system covers.

In the UK, depending on your location, you can get insulin pumps on the NHS. Sometimes priority is given to children, and you may need to do a trial or wait on a waiting list, but it is possible to get one.

In Northern Ireland, they are pretty good with insulin pumps! My advice would be to not give up if they say you have to wait six months, ring them every other day to try and get moved up that list. Persistence is key. If youâre willing to wait, theyâre going to make you wait a long time. So donât.

In the USA, insulin pumps are sometimes covered under your insurance plan. You will need to discuss with your insurer and doctor about what type of insulin pumps are covered and on a lot of occasions, CGMâs are also covered too.

In Ireland, I,e the Republic of Ireland, insulin pumps and CGMâs are covered under the health system, but I donât know what the reality of actually getting one is, so let me know. But you will need to visit your doctor and prove as to why you would benefit from one etc.

As for other countries, please do let me know how it works in your country, there are too many to list them all!

How Automated Insulin Delivery Works

One advantage of an iCGM system like the FreeStyle Libre 2 is that it can be paired with an automated insulin pump, says Roman Hovorka, PhD, of the University of Cambridge Metabolic Research Laboratories in England.

Linking a continuous glucose sensor to an insulin pump can double the benefit by reducing blood sugar, increasing the time that patients have glucose levels in a safe and healthy range, and by reducing the risk of dangerously low blood sugar, or hypoglycemia, Dr. Hovorka says.

Glucose sensing without taking action has little benefit, says Hovorka. Automated insulin delivery allows the cycle measurement-action-measurement to be taken automatically all the time.

Can I Use My Freestyle Libre Reader And Freestyle Librelink App* With The Same Sensor

You can monitor your glucose with either the app, the reader, or both.* You can use the FreeStyle LibreLink app* and the FreeStyle Libre reader with the same sensor, as long as you start the sensor with the reader first, and then scan the sensor with your phone at any time during the sensor life.

Remember that the FreeStyle LibreLink app* and the FreeStyle Libre reader do not share data with each other. If you switch between your reader and the app, you may see gaps in your glucose data, which will affect the reports available through the reader, the FreeStyle LibreLink app* and LibreView. It is important to continue scanning your sensor with either the app or the reader at least once every 8 hours so you and your healthcare professional can get a complete picture of your glucose trends and patterns.
