Freestyle Libre For Type 2 Diabetes

Clinical Inertia Is Not The Fault Of Patients

Freestyle Libre 2 *Unboxing Insertion* | She’s Diabetic

Clinical inertia reflects structural flaws in our current healthcare system, especially when you take into account how many medications and devices exist today to help improve blood sugar management.

For example, diabetes education is limited to only the first year after diagnosis, whereas:

  • Its well-known that many patients struggle with denial during that first year, which means that paid-for education that is limited to the first year is poorly timed.
  • Diabetes education should be an ongoing project between a patient and their healthcare team, giving them years to gradually evolve their understanding and habits around food, exercise, diabetes, and overall self-care.
  • Communication and overall cohesive cooperation between patient, doctor, diabetes educator, and health insurance seldom work smoothly or in unison.

The FreeStyle Libre improves diabetes management in those willing and able to use it, but the large majority of the type 2 diabetes population needs a healthcare system thats designed to guide and support them at a realistic pace throughout their life with diabetes.

Whats The Difference Between Flash Glucose Monitoring And Continuous Glucose Monitoring

Continuous glucose monitors measure your sugar levels continuously and send data to your display device . So you can set alerts for high, low or rate of change. With flash glucose monitoring, you only get your reading and trends when you scan your sensor.

If you want to find out more about CGM, we have loads of information.

What You Need To Know About Diabetes Freestyle Libre Diabetes Patch

One of the most common signs of diabetes is excessive thirst. You may also feel more hungry and pee more than usual. Its important to check your blood sugar as soon as you start to notice these symptoms. If your blood sugar drops below 70 mg/dL, you should immediately eat 15 grams of carbohydrates. Then, check it again fifteen minutes later. If you cant eat that much carbohydrate at once, you can try oral glucose.

Despite the many complications of type 1 diabetes, you can still find ways to manage it. By reading about diabetes, youll be better prepared to live a healthy and happy life. You can avoid diabetes by making healthy choices. In fact, there are many treatments available for type 1 diabetes. In some cases, a patient may even experience an improvement in their overall health after the transplant. In some cases, you can even reverse your diabetes through a simple procedure.

Type 2 diabetes is the most common type. Most people with type 1 diabetes have type 2 diabetes. They both require insulin to regulate their blood sugar. If youre overweight, you may have type 2 diabetes. If youre concerned about diabetes, its important to learn about it. Your doctor can provide you with more information on your condition, including tips on how to cope with it. You should never be afraid to ask questions. Just remember, youll have to answer them. Youre not alone.

Comparison To Other Devices

The FreeStyle Libre is referred to as a flash glucose monitoring system. Unlike other CGMs, like the Medtronic Guardian 3 and Dexcom G6, the FreeStyle Libre checks glucose levels every minute rather than every five minutes. It can also be worn for 14 days as opposed to seven minutes for the Medtronic Guardian 3 or 10 minutes for the Dexcom G6.

The FreeStyle Libre system does not require fingerprick calibrations. And, because there is no transmitter, the system costs less than other CGMs.

Whereas other CGM sensors can be placed on the belly and buttocks, the FreeStyle Libre is only approved for use on the back of the arm. If placed in other areas, the sensor may not work properly.

A newer CGM called the Eversense system offers continuous 90-day glucose monitoring. But, sales of the device were halted in 2020 due in part to the COVID-19 pandemic but also to the fact that the device needs implantation by a doctor.

Diabetes can be managed through oral medications. In addition to insulin, metformin, which is a sugar-reducing agent, is also a common treatment for diabetes. It is considered a first-line therapy for diabetes treatment and is often added to insulin. It is important to know the risks of taking diabetes medications. Some drugs can be addictive, so you must consult your doctor before taking any medication. Your physician can prescribe you an appropriate treatment plan based on your medical history.

Your doctor will prescribe medication and check your blood glucose levels on a regular basis. Your A1c level will be checked every six months and your cholesterol levels will be tested regularly. Your doctor will also look for any signs of retinopathy, which is damage to the nerves in the eye caused by diabetes. You will also be examined for any foot problems. It is important to see a foot specialist regularly. Your feet should be thoroughly inspected for damage to the nerves.

While the first two types of insulin are the most common treatments, diabetes can be treated in a variety of ways. Your doctor may prescribe medications to control high blood pressure, which can protect the kidneys. Other types of medication include aspirin and other types of anti-platelet drugs. If your doctor is concerned about your blood sugar level, you may need to try a different medication. Some medications can cause side effects. Your treatment will depend on what type of insulin you need.

Dont Ignore Diabetes Ask For Help

Due to the expense of wearing the Freestyle Libre, Venice doesnt plan on using it on a consistent basis, but she still finds it helpful in helping her manage diabetes more effectively. As a result of her wearing the device for a month and making changes, Venice saw a change in her A1C. In December 2020, her A1C was 6.4 percent, now its 5.4 percent. She hopes these devices can become widely accessible to people with Type 2 diabetes in her country so they can learn to manage it as best they can. For people who are struggling to manage their diabetes or even discuss having diabetes, Venice says dont hesitate to ask for support.

Dont be afraid to ask for help, said Venice. In my country, diabetes carries a stigma and people tend to hide their condition. I think a lot of it has to do with the costs, both financially and health-wise like facing complications such as kidney failure. I just lost my aunt who had untreated diabetes and developed kidney failure. Last November, I lost an uncle due to diabetes-related complications. Do not be afraid to be open and seek professional help. Do your research, too, to make sure you arent being misled by misinformation. You cant ignore diabetes, but you can learn to live with it.

What is already known about this subject?

  • FreeStyle Libre Flash continuous glucose monitoring is an alternative approach to glucose monitoring, more or less comparable to continuous glucose monitoring independent validation testing has not been performed yet. Its accuracy is inadequately studied.

What are the new findings?

  • Accuracy of FSL-CGM is moderate in comparison to standard glucose measurement techniques, showing lower than actual results in the lower ranges, and higher than actual results in the higher ranges after standardized glucose load glucose measurement results lag in comparison with other glucose measurement techniques. In general, users are satisfied.

How might these results change the focus of research or clinical practice?

  • Results of the FSL-CGM should always be considered in clinical context when in doubt, use another glucose measurement technique to confirm or refute the reported FSL-CGM glucose concentration.

How Automated Insulin Delivery Works

One advantage of an iCGM system like the FreeStyle Libre 2 is that it can be paired with an automated insulin pump, says Roman Hovorka, PhD, of the University of Cambridge Metabolic Research Laboratories in England.

Linking a continuous glucose sensor to an insulin pump can double the benefit by reducing blood sugar, increasing the time that patients have glucose levels in a safe and healthy range, and by reducing the risk of dangerously low blood sugar, or hypoglycemia, Dr. Hovorka says.

Glucose sensing without taking action has little benefit, says Hovorka. Automated insulin delivery allows the cycle measurement-action-measurement to be taken automatically all the time.

What Is The Cost Of Each

CGMs can be costly. Depending on which brand you are purchasing, you may have two or three separate items to pay for. All CGM systems require a prescription in order to buy them.

Dexcom G6s average retail cost is about $400 for the receiver, $300 for 1 transmitter, and $420 for 3 sensors . A transmitter has a 90-day battery life, but the sensors need to be replaced every 10 days. You should ask your provider how often you should get a new receiver, as these are meant to last for a year or longer.

The FreeStyle Libre 14 Day reader costs about $87 without insurance and the FreeStyle Libre 2 reader averages around $130. The sensors for both models cost about $130 for a 28-day supply. Again, youll need separate prescriptions for the reader and sensors.

The study was an open-label randomized controlled trial over ten weeks, with 101 total patients with type 2 diabetes, 53 in the intervention group and 48 in the control group, explains the report.

The intervention group was equipped with the FreeStyle Libre FGM which involves a small patch with a sensor inserted into the subcutaneous tissue of your skin. When scanned, the transmitter reports a glucose level. They were instructed to scan their FGM every 8 hours.

The control group was instructed to continue using traditional glucose testing at least 4 times per day.

Both groups were instructed to use traditional glucose testing if they felt symptoms of low blood sugars.

The Diabetes Treatment Satisfaction Questionnaire was used to evaluate the diabetes treatment regimen in terms of overall satisfaction, convenience, flexibility, understanding of diabetes, willingness to recommend to others, and willingness to continue treatment.

Freestyle Libre Reduces Type 2 Diabetes Hba1c Levels

Newly published research has shown the FreeStyle Libre reduces HbA1c levels for people with type 2 diabetes on either long-acting insulin or non-insulin therapy.

Abbott, the company that makes the pioneering system, unveiled the results at the American Diabetes Association 80th Scientific Sessions, which took place in June.

The study results are similar to outcomes typically seen when adding insulin therapy to treatment regimens, indicating people may be able to manage their glucose levels with sensor-based technology instead of adding insulin.

The real-world results are among the first studies that show use of the FreeStyle Libre system can deliver substantial reduction in HbA1c levels for those with type 2 diabetes, whether using insulin or not Dr Eden Miller, lead researcher

During the trial researchers assessed changes in HbA1c levels in people with type 2 diabetes who were either on long-acting insulin or non-insulin therapy. They analysed HbA1c levels from baseline to six months and baseline to 12 months after initiating use of the FreeStyle Libre system.

The results demonstrated overall lower HbA1c levels associated with the use of Abbotts technology, specifically a 0.8% drop after six months and 0.6% drop after one year of FreeStyle Libre system use clinically significant reductions of average glucose levels over time toward the ADAs recommended A1c goal of 7% for adults with diabetes.

How Does A Flash Glucose Monitor Work

A flash glucose monitor has two parts:

  • A sensor that you wear just under your skin which measures your sugar levels.
  • A reader that you or someone else can swipe over the sensor to get both your sugar level and the trend of your levels. You can also scan the sensor with your phone.

Each time you scan your sensor, you can access the last eight hours of sugar levels. Flash glucose monitoring also comes with software so you can analyse your results and see patterns in your sugar levels.

You generally wear a sensor for about 14 days, after that you need to put on a new one.

Sensors should be worn on the arms only, and we recommend that they aren’t placed over areas with tattoos as this could impact your results.

Does Freestyle Libre Require Prescription

Freestyle Libre 14 day sensor application for diabetes–Will it hurt??

Statistical Methods And Ethical Considerations

Results are expressed as mean or median for normally distributed and non-normally distributed data, respectively. A significance level of 5% was used. Normality was examined with Q-Q plots.

Measurements were also assessed through mean absolute differences and mean absolute relative differences for all ranges.

For the glucose challenge test, repeated measures mixed-model analyses were applied to assess MARD between FSL-CGM and PCA-HK, with subjects as a random effect and type of diabetes as fixed effect. Wilcoxon signed-rank test were applied for paired differences between methods at the different time points, that is, 0, 15, 30, 45, 60, 75 and 90min. Outcomes from FSL-CGM and capillary blood glucose readings were superimposed on the error grids as described by Clarke et al. The point accuracy of sensor-based glucose values versus finger stick blood glucose was determined as per cent within consensus error grid zone A. Bootstrap analyses were performed to determine 95% CIs around the percentage of values within zone A. Values in zones A and B are deemed clinically acceptable, whereas those in zones C, D and E are considered potentially unsafe. Analyses were performed using SPSS and Microsoft Excel 2010 with Analyse-It statistical add-on .
