Level2 equips eligible participants with integrated tools that include a mobile continuous glucose monitor , activity tracker, app-based alerts and one-on-one clinical coaching to help encourage healthier lifestyle decisions, such as food choices, exercise and sleep patterns. In the future, UnitedHealth Group may offer the Level2 model to support people with other chronic conditions beyond type 2 diabetes.

Level2 will be made available to select employers with self-funded plans later this year.

Diabetes Prevention Program

The Medicare Diabetes Prevention Program expanded model is a structured intervention with the goal of preventing type 2 diabetes in individuals with an indication of prediabetes. The clinical intervention consists of a minimum of 16 intensive core sessions of a Centers for Disease Control and Prevention approved curriculum furnished over six months in a group-based, classroom-style setting that provides practical training in long-term dietary change, increased physical activity, and behavior change strategies for weight control. After the completing the core sessions, less intensive follow-up meetings furnished monthly help ensure that the participants maintain healthy behaviors. The primary goal of the expanded model is at least 5 percent weight loss by participants. The National DPP is based on the results of the Diabetes Prevention Program study funded by the National Institutes of Health . The study found that lifestyle changes resulting in modest weight loss sharply reduced the development of type 2 diabetes in people at high risk for the disease.

How Medicare Covers The Medicare Diabetes Prevention Program

Medicare Part B provides one-time coverage for the Medicare Diabetes Prevention Program if all three of these conditions apply:

1. You have a high blood glucose test result including one of the following:

  • Hemoglobin A1c between 5.7 and 6.4 percent
  • Blood glucose of 110-125mg/dL when fasting or a 2-hour plasma glucose of 140-199mg/dL within 12 months of attending the first program session.

2. Your body mass index is 25 or more .

3. You have never been diagnosed with type 1 diabetes, type 2 diabetes or End-Stage Renal Disease . Medicare Advantage plans also cover the Medicare Diabetes Prevention Program. Talk to your plan provider for coverage details.

Earlier this year, Livongo announced a partnership with DexCom to sync data from its connected glucose monitors with its platform.

Connected devices such as continuous glucose monitors and activity trackers may be game changers for the approximately 30 million people2 nationwide living with type 2 diabetes.     How To Participate

    Step 1: Get tested.

    Early diagnosis is important. Prediabetes can often be reversed through lifestyle and diet changes, but if left untreated it may develop into type 2 diabetes. To find out if you may be prediabetic, ask your primary care physician if bloodwork is needed for you.

    Step 2: Confirm your eligibility.

    To participate in the program, you must meet the following requirements:

    • Be at least 18 years old
    • Have a body mass index of at least 25
    • Have no previous diagnosis of type 1 or type 2 diabetes
    • Have received a blood test result in the prediabetes range in the past year, scored 5 or higher on the prediabetes risk test, or have been previously diagnosed with gestational diabetes

    You cannot participate in the program if you:

    • Have already been diagnosed with either type 1 or type 2 diabetes

    Step 3: Enroll in the program.

    If you meet the eligibility criteria, contact your CalPERS health plan to participate in the diabetes prevention program.
